Description of Event

On September 27,1980, at 0230 in the morning, it was observed that the pressurizer level transmitter LI-1461 was reading > 3.5% from the average of the transmitter readings, rendering the instrument inoperable.

Upon review of the log books it was discovered that this situation had been in existence since 1400 on 9/26/80. The applicable T.S. is 3.3.1.1 and the event is reportable pursuant to T.S. 6.9.1.9.b and T.S. 6.9.1.9.c.

Probable Consequences of Occurrence Since there were two redundant protection channels available, and 1

LI-1461 was placed in the tripped mode within one hour of discovery, the health and safety of the public were not affected.

Cause of Event

The pressurizer level transmitter failed high because of a failure in the temperature compensator on the differential pressure unit.

Evidently, voids developed in the fluid in the temperature compensating device, causing it to respond erratically.

Immediate Corrective Action

i The immediate action was to place the channel in the tripped mode, and instruct operations personnel to peruse the log records more carefully.

i Scheduled Corrective Action A decision was made to replace the transmitter, bellows, and capillary.

i Actions Taken to Prevent Reccurrence The above action has been completed, and the level transmitter restored to operable status.
