YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 - QUALITY ASSURANCE AUDIT M80-7 DEFICIEUCIC3 2, 3, AND 5 - FIRST I!!rERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector M. Thoman en September 23, 1980, in cocordance with 10 CFR 50.55(e).

Wo expect to submit our next report by November 25, 1980.

The followi7g are construction quality assurance audit deficiencies which are considered significant conditions adverse to quality. TVA is evaluating these deficiencies.

(2) Contrary to 10 CFR 50, Appendix B, criterion V, activities affecting quality are not being accomplished in accordance with the established instructions, procedures, or drawings as evidenced by the Quality Trend Analysis Report of Audit Items YC-TAAI-80-01 of April 1,1980, and TC-TAAI-80-02 of July 1, 1980. The major cause is failure to fol-low procedures /instructicns. Loss o.f operational and procedural control persists as the basic trend.

(3) Contrary to 10 CPR 50, Appendix 3, Criterion XVI, conditions adverse to quality have not been prompt 1/ corrected as evidenced by Quality Trend Analysis Report TC-TA-80-01 dated July 11, 1980. Problems with embedments and/cr rebar continue to increase.

(5) Contrary to the requirements of CAPP-17 Rev 0, Quality Assurance Records, the inspectors performing the inspections are not the ones signing the inspection records.
