O NOTICE OF VIOLATIOh 4
Based on the results of an NRC inspection. conduct August 1-31, 1980, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below-l Technical Specification 7.4.a, Procedures, Administrative Controls, requires in part that " written procedures shall be established, implemented, and maintained...."
Administrative Procedure (ADM) No. 28 for Data Logs, Data Sheets and Charts, Section 3.2, states in part that, "When the responsible person changes a chart or recorders, and once per shift, be sure that the recorder is inking.
At midnight stamp each chart with th* new date.
If the recorder or integrator is not working properly, write a Plant Trouble Report (PTR).
Administrative Procedure P-1, Plant Operations, which went into effect August 4, 1980 states in part "PTR's are initiated for any deficient recorders or integrators.

Contrary to the above, Rod Position Recorder ZR-1233-1 was found not to be working properly on August 6, 1980.
The recording chart was checked and the recorder was found not to have been working since July 27, 1980.
The chart had been date stamped daily, however, no PTR had been written.
This in an infraction.
