On' August. 12, 1980, the-IE inspector discovered, during a review of the Expansion. Anchor Los'(Sheets 19 through 41), that these. tension tests had not been performed on' installed expansion anchors as required by the,above procedure;'and furthermore, these. tension tests had not been performed at
