Arkansas Nuclear One - Unit Two Reportable Event No. 50-368/80-072

Dear Mr. Seyfrit:

This letter is to confirm the conversation between Mr. Larry Bell, ANO Unit Two Operations Superintendent, and Mr. Bill Johnson, ANO Resident NRC Inspector, of 23:00, 9-3-80.

This is reported in accord-ance with Unit Two Technical Specification 6.9.1.8.b.

On 9-3-80 a review by the ANO NRC Resident Inspector of a Containment Cooling Unit surveillance test performed on 8-20-80 revealed inadequate service water flows through the Containment Cooling Unit coolers. The previous reviews of the test data performed by plant personnel failed to properly identify the out-of-spec condition of the service water flow rates.

An immediate re-test and investigation was begun.

Investigation re-vealed severe plugging of the service water flow instrument sensing lines and instrumentation. Action was taken to clear the flow instru-ment sensing lines and check calibration of the flow indicating instru-ments.

A re-test of the Containment Cooling Units was completed with instru-ment taps and flow indicators verified operable. The re-test did not, however, indicate sufficiently increased service water fic,w rates to meet Technical Specification requirements.

The ANO Resident NRC Inspector was notified of the plant condition and the entry into Tech. Spec. 3.0.3 @ 23:00, 9-3-80.

A shutdown was begun @ 23:05. NRC headquarters was notified of the event @ 23:07.

Investigation is continuing. A follow-up report will follow.
