' Condition 1 - Ucensee sho!! file with the Customs Officer or the Postmaster two copies,in addi-
- tion to those ot_herwise rec;uired, of the Shipper's Export Declarction coverb.g
- ecch export end mark one of such copies for trcnsmittel to the U.S. Nuclear Regu-
-letory Commission, Washington, D.C. 20555. The following dec!cration should cc-company or be pieced on the Shipper's Export Declerations for such exports:
"This shipment is being~ made pursuont to specific license number (specific license number) filed at(location of Custcms office where license is filed),
on (date license ~was filed). This license expires. on (expiration date-of license), and the unshipped bolence remcining on this license is sufficient to cover the shipment described on this declcration.'
Condition 2 - Exports authorized in any country or destinction, except Country Groups O, S, W, X, Y, and Z in Part 370, Supplement No.1, of the Comprehensive Export Schedule of the U.S. Depcrtment of Commerce.
Conditio.13 - This license covers only the nuclear content of the meterici.
Condition 4 - The material to be exported under this license shcIl be shipped in cccordance with the physicci protection recuirements for special nuclecr meterict in 10 CFR 73.
Condition 5 - Specici nudecr materici cuthorized for export under this license shall not be trcnsported outside the Umted States in pcssenger carrying circraft in shipments exceeding (1) 20 groms or 20 curies.'whichever is less, of plutonium er uranium 233, or (2) 350 grcms of urenium 235.
Condition 6-This licen:e authorizes export only and does not cuthorize the rece;pt, physicci possession, or use of the nuclear metencl.
Condition 7 - The licensee shcIl complete and submit en NRC Form 711 for each shioment of source materici exported under this license.
Condition 8 - The licensee shcIl cdvise the NRC in the event there is eny change in the designe-tion of the compcny who will package the nuc!ecr meterici to be exported under this license, or cny chcnge in the locction of the packcging operation, et least three weeks prior to the scheduled dere of expert.
