Cause of Event

Personnel-involved in the performance and review of the' surveillance test failed to notice that'the pump differential pressure was above acceptable limits.

Immediate Corrective Action

Immediate upon discovery of the unacceptably high pump differential pressure, the test was reperformed and the results were satisfactory.

i Scheduled ~Correcti'.e Action

~

Due to the failure of the surveillance test, test frequency for casing cooling pump 1-RS-P-3A will be ~ doubled as per the instructions in ASME Section XI,-subsection IWP.

Actions Taken to Prevent Recurrence Personnel involved in the performance and review of surveillance tests have been informe/ to be aware that a more complete review of surveillance tests is required.
