CONDITIONS PRIOR TO OCCURRINCE The senior reactor operator was supervising a shift as a senior licensed foreman.
DESCRIPTION OF OCCURRENCE On June 5, 1975 it was discovered that license No. SOP-1748-1 (Raymond J.
Christensen) had expired. License No. SOP-1748-1 has an effective date of December 8,1972 and an amended date of June 28, 1973 The license renewal application was subc:itted June 4,1975. The applicant has signed five te=porary procedure changes since January 1975 8009150 Q h

Di!iSCRIPTION OF APPARDrP CAUSE OF OCCURRENCE The license was inadvertently allowed to lapse because the facility and the licensee used the amended date rather-than the effective date as a basis for license renewal.
ANALYSIS OP OCCURRDCE There were no safety implications since the licensee had been functioning as a senior reactor operator during the licensed term and had duly requalified each year.
CORRECTIVE ACTI M The licensee has submitted a license renewal application. He will not be allowed to function as a senior reactor operator until the renewed license is received. Other licensed operators were reminded by newsletter that renewal applications must be based upon the effective date offthe license.
