lll Appendix A NOTICE OF VIOLATION Michigan State University Docket No. 50-294 Based on the inn ection conducted on August 11-13, 1980, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. These items are infractions.
1.
10 CFR 55, Appendix A, Item 4.a requires " Annual written examinations which determine areas in which retraining is needed....."
Contrary to the above, the 1978 examination was conducted in February, 1979, and the 1979 examination during May, 1980, exceeding the annual frequency requirements.
2.
Technical Specification D.5. requires the reactor to be pulsed semi-annually; E.12.b. requires semi-annual tests;
- b. control rod drop times and E.13 requires semi-annual inspection and maintenance of the pulse rod drive cylinder.
Contrary to the above, the reactor was not pulsed between March 9, 1979, and November 12, 1979, a period of eight months.
The control rod drop times were not determined between December,1978, and September, 1979, a period of nine months; and the pulse rod inspection was not carried out between December, 1978, and September, 1979, a period of nine months, i
