The following modification is made to the' contract at the total estimated cost of $15,812. 00.

A.

Article I - Statement of Work is modified as follows:

1 Work Requirements - Task 1 and Task 2 is revised by deleting 4

" Target Completion Date:

21/2 months after award of contract" and

" Target Completion Date: 61/2 months after award of contract" respectively.

2 "This' contract shall be extended to include task 2 at the unilateral option of the Government. The Contracting Officer shall provide written notice of extension to the Contractor within the period of performance specified in this contract if the Government choses to

exercise this option.

Prior to issuing this notice, the Contracting Officer shall have given preliminary notice of the Government's intention to extend this contract at least thirty (30) days before the contract is to expire.

(Such a preliminary notice shall not be deemed to commit the Government to extention.) The total duration of this contract including the exercise of the option stated under this Article shall not exceed 15 months. Should the Government exercise the option hereunder, all contractual terms and conditions shall apply during the option period."

Meetings and Travel - The second paragraph is deleted and the following is substituted:

"The contractor shall visit six nuclear power plant sites and two similators for the purpose of discussing training and other related issues regarding the procedures. The plant selections shall be agreed upon betwee'n the NRC Project Officer and the contractor."

4 Reporting Requirements is deleted and the following is substituted:

Interim Reports -

Reproducible interim reports and three copies containing the preliminary study results of all plant sites based on the procedures furnished to the contractor by NRC shall be submitted prior to the NRC, site reviews.

Eight reproducible draft reports containing the results of the preliminary study of the site and the review and evaluations performed during the six nuclear power site visits and two simulator visits by the contractor shall be submitted to the NRC Project Officer no later than one week subsequent to the visits.

b.

Fina'l Report -

The final report shall contain a compilation of all twelve plant reviews plus recommendations for further improvements to the procedures.

For the plant site in which the contractor does not visit, the NRC may provide revised procedures.

The contractor shall include their evaluation of these revised procedures in the final report.

The contractor shall submit the final report to the NRC project officer no later than November 21, 1980.

The NRC project officer will furnish comments on the draft final reports no later than December 16, s.

A reproducible final report and ten copies, incorporating NRC's comments, shall be submitted to the NRC project officer and one copy shall be submitted to the Contracting Officer no later than December 31, 1980. The final report shall be prepared in accordance with Appendix B. NRC Manual Chapter 3202.

Task 2 (0PTION) -

a.

A reproducible draft report and three copies containing the recommendations and the basis upon which the recommendations were developed shall be submitted to the NRC project officer no later than April 1,1981.

The NRC project officer shall furnish comments on the draft no later than May 1,1981. A reproducible final report and ten copies incorporating NRC's comments shall be submitted to the NRC project officer no later than May 20, 1981.

b.

This final report shall be prepared in accordance with Appendix B, NRC Manual Chapter 3202.

3.

Monthly Business Letter -

A monthly business letter report will be submitted by the 15tn of the month to the cognizant branch chief with a copy to the cognizant Assistant Director and Technical Support Branch, ATTN:

B.L. Grenier.

These reports will contain the following information related to the accomplishment of this program, a.

Summary of the progress and work completed during the period, including milestones reached or, if missed, an explanation provided; b.

The amount of funds expended for manpower and computer during the period and cumulative to date; c.

If problems are encountered or anticipated, a description of the plans for their resolution, the schedule of their implementation and their impact on the overall program; and d.

Plans for the next reporting period.

B.

Article II - Period of Performance is revised by deleting "for a Period of 61/2 months" and substituting "through December 31, 1980."

C.

Article III - Government Furnished Material is revised by deleting "on the effective date" and substituting

" periodically throughout the term.'

Paragraph 1 is revised by deleting "$78,224", "$72,430" and

"$5,794" and substituting "$94,036", "$87,071", and "$6,965",

Total funds currently available for payment and allotted to the contract for the performance of Task 1 are $58,163, of which $53,855 represents the estimated reimbursable costs, and of which $4,308 represents the fixed fee.

Funding for Task 2 will be made available only upon the decision of the Government to exercise the stated Task 2 option.

For further provisions of funding, see the General Provisions, Clause 3 entitled " Limitation of Funds."

Paragraph 3 is revised by deleting "21/2 months from the effective date of the contract" and' substitute "no later than December 31, 1980."
