,b Appendix A NOTICE _OF VIOLATION Commonwealth Edison Docket No. 50-295 Company Docket No. 50-304 Based on the inspection conducted on August 4 and 5, 1980 it appears that one item was in noncompliance with NRC requirements, resulting in a-deficiency, as noted below.

10 CFR 50.54 (i-1) requires that the licensee have in effect an operator requalification program. Commonwealth Edison Procedure, (ZAP 2-52, Appendix A, Rev. 1, 1-12-79 -Requalification Program for Licensed Operators), requires an Operator or Senior Operator, re-ceiving a grade of less than 80% on one examination subject of the requalification examination, attend a lecture on that subject, followed by a quiz to demonstrate comprehension.
Contrary to the above, twelve operators who had received a grade of less than 80% on one examinstion subject of the requalification examination did not take the lecture quiz and ten of these twelve did not attend the specific lecture on the subject in which they received a grade of below 80%.
