As a result of the ongoing investigation concerning inadvertent isolations of the High Pressure Coolant-Injection-(HPCI) System due to high steam line flow when the HPCI is started, the plant discovered that the steam line flow instrument differential pressure setpoint was too low. There#cre, the steam j

flow required to start the HPCI turbine periodically caused system isolations.

1 Since previous repairs did not seem to be completely effective, a more indepth investigation was started and the system maintained in the inoperable I

condition as allowed by the 7-day Limiting Condition for Operation (LCO) in l

the technical specifications while the cause was being thoroughly investigated.

The LC0 expiration was 6:00 am on December 12, 1989.

I The investigation led to evaluation of the method used to determine the l-correlation between the desired 300% steam flow isolation setpoint and the '

l di'ferential pressure which would result from this flow.

Determination of l

this differential pressure was previously addressed in Amendment No. 68, issued l

March 4, 1982. However, when Revision 2 of GE SIL No. 475 was used, the l

actual flow setpoint was found to be approximately 200%. This SIL, which was i=

issued on November 28, 1988, contained a more accurate method of determining l

the differential pressure setpoint which corresponds to 300% steam flow, based on actual plant data measurements.

i Due to HPCI control system modifications performed during the 1989 maintenance l

outage, this condition only recently started causing isolations when higher steam flows were required to start the system.

Licensee to implement the new setpoint. This has resulted in declaring the system operable following a successful retest prior to expiration of the-lLCO. The technical specification change is being processed on an emergency basis.
