4 December 11, 1989 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PNO-ADSP-89--07 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all-that is known by the ADSP staff on this date.

SUBJECT:

INADVERTENT SAFETY INJECTION SIGNAL At 12:14 p.m. (CST) on December 7,1989, an inadvertent safety injection signal was received during testing of the solid state protection system due to a procedure deficiency.

Pumps and diesel generators did not start since they were administratively disabled. Valves repositioned as required.

Reactor vessel water level rose about 1-1/2 inches by gravity flow from the RWST.

Systems were returned to normal using emergency procedures. This PN is being issued due to local media interest.

The applicant is investigating the incident and revising procedures.

Local media interest was evident on Decenber 7 and 8,1989. The NRC does not i

plan to issue a news release. Both the NRC and the applicant responded to press inquiries.

The State of Texas will be informed.

The Comanche Peak Project Division received initial notification of this occurrence by telephone from the applicant on December 7, 1989. The information presented herein has been discussed with the licensee and is current as of 8:00 a.m. (CST) on Decerber 11, 1989.
