Dear Mr. Grier:

'NRC INSPECTION REPORTS 50-354/80-04 AND 50-355/80-04 NO. 1 AND 2 UNITS

' HOPE CREEK' GENERATING STATION We have received and reviewed the report of your-inspection conducted

-on March 31 - April 30, 1980. This was transmitted with your letter of June 18, 1980 and received on June 24, 1980. The following cor-rective actions have been taken relative to the infraction determined.

Infraction:

10CFR, Appendix B, Criterion X states, in part, that: "A program

for inspection of activities affecting quality shall be established

'and executed by ... .the organization performing the activity to verify conformance with the documented instructions, procedures, and drawings for accomplishing the activity."

Section 16.4.10 of: Chapter 16 ~ of' t.he PSAR states, in part, that:

" ... Field Quality Control Engineers perform inspections in accordance with'~the Master Inspection-Plans."'

Paragraph 2.2.a of Quality: Control Instruction C-1.30, Rev. III,

" Concrete Placement-Inspection," states: " Free drop of concrete shall not . exceed 6 feet unless approved tur Field Engineering.

Concrete shall not be dropped through dense-reinforcing steel.

In such cases chutes, trunks, or other appropriate methods should be'used."

Mr. Boyce'H. Grier 7/21/80 Contrary to the above, as of April 15, 1980, QC fa_ted to execute the inspection program for pour 1B-F/G-W-903 as was evidenced by their awareness of and lack of response to concrete being free-dropped in. excess of 6 feet through dense rebar.

This item is an infraction applicable to Docket Number 50-354.

Reply to Infraction:

1. Corrective steps which have been taken by Bechtel and results achieved:

Discussions with the Responsible Field Engineer following the placement indicated that he would have approved the dropping of a small quantity (two plus cubic yards) of concrete in excess'of 6 feet had objectionable segregation not occurred.

While the conditions mentioned in the violation increase the probability of segregation, no objectionable segregation occurred in the opinion of the Quality Control Engineer and as evidenced upon the removal of forms in the area.

2. Corrective steps which were/will be taken to avoid further items of noncompliance:
a. Further training of all Civil QCE's accomplished regarding-their actions when items are not within compliance to Specifications and Drawings, including QC's Stop Work Authority and the execu-tion of the same.
b. All Superintendents and Field Engineers directly associated with concrete operations were instructed to review Concrete Specifications and Procedures and were told strict compliance to these is mandatory.

They were also instructed that if conditions existed making it impossible to physically comply, Field Engineer-ing is to be contacted immediately for relief as prescribed in the Specification.

3. The date when full compliance will be achieved:

Full compliance for this item has been accomplished per training performed.
