HARTSVILLE AND PHIPS BEND SUCLEAR PLANTS - REPORTABLE DEFICIENCY -

RPS PANEL DESIGN DEFICIENCIES (NCR PRC 80-31)

Initial report of the subject deficiency was made to NRC-0IE, Region II Inspector, F. S. Cantrell, on August 7, 1980.

In compliance with paragraph 50.55(e) of 10 CFR Part 50, we are enclosing the first. interim report for the subject deficiency.

TVA anticipates transmitting the final report on or before November 19, 1980. TVA considers 10' CFR Part 21 applicable to this reportable deficiency.

NUMBER PRC 80 ~ 0n August 7,1980, TVA informed NRC-0IE, Inspector F. S.'Cantre,ll, of a reportable condition under 10CFR50.55(e) regarding design-

deficiencies'in NSSS Nuclear-System Protection-System (NSPS) power

-- distribution. panels'. This condition was reported. separately by General Electric (GE) to NRC-0IE Region V. Inspector R. H. Engelken, under 10CFR21 on August 6,1980.' This is our first ~ interim report on the srbject. reportable condition. Our final report will, be submitted on or before Nove,mber 19,-1980.

Description of Deficiency GE, the manufacturer and supplier of the NSPS power distribution

. panels'found, during a design review of the panels on August 16, 1979, seven safety related design deviations from Regulatory Guide 175 These deviations occur. in NSPS panels C71-P011 wherein four divisions of power plus nonessential power are used in the panels without proper isolation.. Regulatory Guide 1 75 requires isolation in the form of physical independence or separation of power supplies to prevent hot shorts that may lead to'the loss of capability to scram.

These panels.were placed on " Hold" when the deviations were identified. 'However, these panels.for Hartsville units A1, A2, and B1 (one panel ~per unit) were shipped to the site without a conditional release (in violation of the." Hold").

The-. panel for Hartsville Unit B2 had been fabricated but was not shipped. The panels for both Phipps Bend units were still in the fabrication stage.

Safety Implications

=. E Failure to provide proper isolation e' essential power supplies could lead to-the possible loss of the capability to scram, thereby, jeopardizing the. safety of operation "of th.e plants.

Corrective ~ Actions e

GE issued Field Disposition Instructions (FDI's) to Hartsville requesting thel return of the. three deficient NSPS power supply panels discussed above.

These panels were returned to GE, San' Jose, for.

further' corrective actions including redesign and rework to the corrected design.
