. NOTICE-OF DEVIATION l
l Based on the results of an NRC inspection conducted on June 2-4, 1980, it

appears that cetain of your activities were not conducted in accordance with j
NRC requirements ~as. indicated below:
l' Criterion-V of Appendix B to 10 CFR 50 states:
" Activities affecting quality i
shall be prescribed by documented: instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be.. accomplished in accordance l
with.these instructions, procedures, or drawings.
Instructions,' procedures, or drawings shall1 include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviation ~from this requirement is as follows:

Manual, Section 7.0, sub paragraph 7.2, Controls and Procedures, states in part, l

7.2.1 Environment, " Utilization of measurement standards shall be in an office or laboratory environment.
' Contraty to the above the following was observed by the inspector, l
-Calibration of measurement tools, using measurement standards was being performed in'the tool room area, not in an office or laboratory environment.

.7.2.5 Records, " Records of calibrations shall be maintained by the Quality Control Department....
LThe records shall be maintained l-on a Tool.and Gage Records Card which shall present individual records of calibration, calibration intervals, dates of individual records of calibration, actual values, procedures with revisions, serial number of standard used, and due date for next calibration
' Contrary to.the above.the.following was observed by the inspector, Calibration records of equipment was not being maintained on a Tool and Gage' Records' Card as required by the.QA Manual.

7.2.6 Status of. Calibration. " Measuring and test equipment shall
'be labeled to indicate date of last calibration, by whom calibrated,-and date of next calibration due.
Labels are affixed:directly to the case or container whenever.the individual piece cannot be labeled."

G&W Taylor-Bonney Division Contrary to the above the following was observed by the inspector, Five (5) welding machines were found to have calibration labels showing incorrect data. Equipment had been calibrated as required but new labels had not been attached.

7.3 Tool and Gage Use Records, " Tools and gages shall be issued to an inspector by number as recorded on a Tool Loan Slip and Log.
If not returned by due date of next -calibration, Quality Assurance Department shall locate tool or gage, tag and remove from service."
Contrary to the above the following was observcd by the inspector, Tools and gages were not being issued as required.
A Tool loan slip and log has not been used in over three (3) years.
