Dear Sirs:
We hereby report, subject to Technical Specification 6.7C, a failure to com-ply with 10 CFR 55.3 during the period from 27 June 1980 through 18 August 1980.
An application for renewal of license SOP-3278 was submitted on 24 April 1980, with the medical examination form riiled separately on 22 May 1980. Since the license expired on 27 June 1980, we -nsidered the submission to be a timely renewal and that the license would not . sire until final commission action (as stated in 10 CFR 55.33(b)).
On 19 August 1980, we received a .enewal of another operator license for which a renewal application had been mailed on 3 June 1980 (expiration date 12 July 1980).
A telephone call to the NRC Division of Human Factors S.fety on 12 July 1980 led to the determination that that office had on file tb medical exam form but not the application for renewal of license SOP-3278.
' Upon determination that the license was not in the process of renewal, we took the following actions.
(1) Notified the individual and remaining opera .ing staff that his license was no longer valid and that he cot Ed not carry out licensed duties until final action on his ienewal application.
(2) Submitted photocopies of the application t the appropriate branch of NRC for action.
