Dear Mr. O'Reilly:

On July 30, 1980, Mr. Bob McFarland of NRC-0IE Region II was notified of a potential significant deficiency under the provisions of 10CFR50.55(e) regarding lack of full penetration welds for attach-ments to Class I pipe. This is a violation of design requirements.

These welds were made by ASME qualified welders using approved procedures. The welds were examined by magnetic particle or liquid penetrant methods in accordance with approved procedures; however, the lack of full penetration was uncovered only after these welds were radiographed. The film revealed incomplete penetratica and/or incomplete fusion.

This finding has been documented on site (NCN-2137) and will be dispositioned by reworking the Class I attachment welds to provide full penetration as required. Inspection will be verified by RT or UT that full penetration welds have been achieved.

In addition to_the above corrections for the Class I lines, Class II and Class III full penetration pipe attachments will be inspected and evaluated, including both field and shop welds. The evaluation will consist of measuring the actual fillet reinforcement of welds to determine whether sufficient weld metal is available to carry-the design loads.
