O APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Based on the results of an NRC inspection conducted on April 28 - May 1,1980, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC Facility License No. DPR-28, as indicated below.
This item is a deficiency.
Criterion XVII, Quality Assurance Records, of Appendix B to 10 CFR 50 states, in part:
" Inspection and test records shall, as a minimum, identify the inspector or data recorder, the type of observation, the results, the acceptability and the action taken in connection with any deficiencies noted".
The approved procedure, OP 4424, Revision 5, Cutrol Room Scram Testing, issued Septenter 14, 1979, requires "3.a Record reactor power, reactor pressure, time, date and accumulator pressure on VYOP 4424.01".
Contrary to the above, data sheets VYOP 4424.01 for Scram #94 occurring at 2200 on November 19, 1979, did not contain the required reactor power, reactor pres-sure and accumulator pressure data results.
