NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on May 12-16, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting qaality shall be prescribed by documented instruc-tions, prccedures, or drawings, of a type appropriate to the circum-stances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accom-plished."
Deviations from these requirements are as follows:
A.
The General Atomic Quality Assurance Manual, dated March 20, 1980, section 6, paragraph 2.2.1 states in part; "Esch product document shall be:
(a) reviewed for adequacy prior to release (d) distributed to the location where the activity is performed (e) Used at the location where the activity is performed."
Contrary to the above, some product documents were not adequately reviewed prior to release in that six of the seven procedures inspected were inadequate in some measure.
One of the inspection procedures did not have the current revision distributed to the work location.
One of the manufacturing procedures was not used at the work location in that the primer set time was not held by manufacturing (See Details, paragraph C.3.a.(1)).
B.
The General Atomic Quality Ausurance Manual, dated March 20, 1980, section 12, paragraph 2.D states in part;

" Cages, instruments, measuring, recording, and testing devices, and special tooling used for inspection and acceptance and for other activities affecting quality shall be controlled..
Measuring instruments shall be identified and controlled by use of a serial number assigned to the instrument."

Contrary to the above, the cure oven chart recorder's drive mechanism was not controlled or calibrated.
The thermocouples used with the cure oven chart recorder were not identified with a serial number (See Details, paragraph C.3.a.(2)).
C.
The General Atomic Quality Assurance Manual, dated March 20, 1980, section 5, paragraph 2.5, states in part;
" Instructions for work affecting quality shall provide appropriate quantitative or qualitative acceptance criteria for determining that the work has been accomplished in accordance with the specified requirements."
Contrary to the above, some packaging requirements contained in the design specification, GA10600, were not included or referenced in inspection and at,ufacturing procedures (See Details, paragraph D.3.a.(1)).
D.
The General Atomic Quality Assurance Manual, dated March 20, 1980, section 13, paragraph 3.1, states in part; "The responsible design engineer shall define the handling, storage, and shipping measures necessary to prevent damage to, or deteriora-tion, loss, or substitution of items to be delivered to the Customer or his agent."
Contrary to the above the design documents did not address handling i
measures necessary to prevent damage.
