Midland Proceedinc - Unit 1 Tendon Sheathine Gentlemen:

Enclosed is the final report on the misplacement of the tendon sheathing for Midland Unit 1.

As you will note, the repair has been completed and the Bechtel nonconformance has been closed out.

This report completes the documentation required by the NRC on this item.

There was no impact on the construction schedule as a result of this repair.

When the final cost of the repair is obt:ained, I will supply you with that informatien.

When the final report on the Unit 2 liner bulga is completed, I will provide the Board with a copy.cf that report.

References 1 and 2 vere interis repohs which provided a dc.scription of the deficiency, an analysis of the safety implications, and the proposed method of repair.

This is a final" report and includes corrective actions taken to preclude recurrence of a s4 41ar situation.

The Nuclear Regulatory Co==ission was advised of the repair =ethod in an interi=

report (attached to Reference 2) and in a tech '. cal =eeting in Ann Arbor on June 30, 1977 Reference 3 documented both the June 29 URC inspection of the repair area and the NRC participation in the June 30 neeting in Ann Arbor where it was agreed that Consumers Pcver could proceed with the repair. The repair was cc=pleted on July 22, 1977 and Bechtel Nonconfor=ance (NCR) 778 vas closed out at that time.

Corrective action taken by Bechtel Fover Corporation is docu=ented and verified in Reference h and its attach =ent (both enclosed).

Midland Tendon Sheathing Final Report August 12: 1977 In addition to the 3echtel corrective action, Consu=ers Power Cc=pany expanded its inspection progra= to include all =-'ad ents for Class 1 structures pricr to release for concrete place =ent.

This is consistent vith the Cons==ers Power Co=pany ec--4tnent documented in ite: 3 of Reference 5.

Also, there has been an on-going joint effort by Const=ers Power Cc=pany and Bechtel to reevaluate all 3echtel Quality Control Instructions (qCIs) with the objective of i= proving the co=pleteness and specificit/ of inspection procedures and acceptance criteria.

Reevaluations have been ec=pleted for all QCIs ccver-ing first-line inspection activities. and d' afts of these revised QCIs are being re-reviewed with a target date of Septe=ber 15,197T for their final approval and inple=entation.

Eovever, the QCI covering the installation of post-tensioning ec=ponents (C-9.00) had been revised and received final approval.on April'20, 1977, incorporating the corrective actions indicated in Reference h.

In view of the above, Consu=ers Pcver Cc=pany believes adequate and cc=prehensive corrective action has been taken and sub=its this as a final report in this =atter.

Bechtel Letter v/ attach =ent to CPCo, BLC-4079, dated 5/10/77 The subject MCAR is now closed based on conclusions, per Reference A, that the original design criteria could be =et by rerouting one sheathing from below to above the penetration with ninor align =ent adjust =ents to the sheathing directly above.

Align =ent of the three sheathing directly below tha penetration will also be revised. An evaluation by li!ruCCO indicates th_c the friction losses in the affected tendons have been reduced due to this revised align =ent and the capability of the contain=ent structure to resist forces has been i= proved.

Strength losses due to cuts in the penetration gusse't plates and the cut-offs of rebars, both necessitated by the rerouting of the tendon sheathings, have,been fully replaced by the =odifications discussed in Reference A.

Therefore, there is no adverse safety i= plication.

Corrective actions have been i=ple=ented to preclude recurrence of misplaced tendon sheathing as indicated in Reference I.

These include requirc=ents for:

1.

Survey notes to receive initials verifying position of each individual sheathing.

2.

Field Engineering to take a copy of the sheathing drawings with thc= during installation and check off each sheathing for corrcet configuration.

Quality Control to = ark up a sheathing drawing during place-ment checkout to verify correct configuration-and nu=ber of sheathing.

Additionally, Quality Control and Construction Division !!anagers' discussions with all Bechtel field ccustruction persennel vera conducted e=phasizing the responsibilitics of construction quality control, field engineering and super-vision at the 111dland jobsite.

Bechtel QC has ex'e=plified the policy of stressing inspection acceptance be made on the basis of evidence which clearly de=onstrates that the harduare is in exact agres=ent with applicable drawings and specifications by revi-sions to QCIs.

Bechtel QC is kept current on schedule changes by area Supervision and Field Engineering personnel as part of their nor=al coc:=:sications.

Additionally, QC personnel attend daily scheduling =eetings where changes to the published three week schedule are discussed.

The i=portance of this cor=unication. flow is continually stressed to all concerned parties.
