Dear Mr. Grier:
This letter is forwarded to provide prornpt notification of Reportable Occurrence RD-80-11/IP as required by Millstone Unit i Technical Spect ffcations.
On August 3,1980 at 0215 hours0.00249 days <br />0.0597 hours <br />3.554894e-4 weeks <br />8.18075e-5 months <br />, while investigating the. reason for Tow off gas sample fTow and radf ation level, it was discovered that the off gas delay pipe sanple Ifne nanual stop valve was closed.
The delay pipe had been placed in service on 8/2/80 at 2235 hours0.0259 days <br />0.621 hours <br />0.0037 weeks <br />8.504175e-4 months <br /> following minor operating difficulties with the off gas reccachiner syster.
With this valve closed and the off gas delay pipe in service, a high off gas condition wuld not have resulted in closure of the off gas isolation valve.
Upon realtzation of the probTem, the sample If ne stop valve was opened and nomal off gas monitoring was restored.
At the time of the occurrence the plant was operating at steady state full power.
Results of the investigation into the incident and further analysis will be included in the follow-up T4 day report.
