In accordance with the requirements of Appendix B to Reference (a), we have enclosed a check in the amount of Four Thousand Dollars ($4,000.00) as payment of the civil penalty identified therein.
As directed by Appendix B, our response to Appendix A, Notice of Violation, will be transmitted under separate cover.
