[7 Appendix A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-329 Docket No. 50-330 Based on the results of NRC inspections on August 30 through September 2, and September 13 to 15, 1977, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. The items below are considered infractions.
1.
Contrary to the requirements of 10 CFR 50, Appendix B, Criterion V, and Policy No. 5 of the Consumers Power Company Quality Assurance Program Topical Report (CPCQAPTR) CPC-1, CPC failed to assure that their contractor, U.S. Testing, followed their procedure UST-Audit-1, Revision 3.
2.
Contrary to the requirements of 10 CFR 50, Appendix B, Criterion IX, and Paragraph 1 of Policy No. 9 of the CPCQAPTR-CPC-1, it was determined that the Nelson Stud welding procedure F1-Stud C3 Welding Revision 2, dated July 29, 1976, was not qualified by their contractor, Bechtel, prior to its use in the field.
3.
Contrary to the requirements of 10 CFR 50, Appendix B, Criterion X, and Paragraph 1.c of Policy 10 of the CPCQAPTR-CPC-1, the acceptance and rejection criteria for the size and length of welds were not established in QC inspection checklists.
