AS !0ctAL CCCURREliCE REPORT TO C0 TIGRESS - SEC0!!D QUARTER CY 1978 Pursuant to your subicct recorandura dated June 23,1978, we have rvviewed the MPA request for inputs to the report.
Items 1 through 3 are addressed below:

We have already identified events other than those listed wnich we believe should be censidarcd as possible abnomal occurrences. These events are " Diesel Generator Autoratic Loading Sequence Failure' at Devis-Casse Pc<:er Station, Unit 1, and " Crack In The Ten-Inch 7ecirculatTcn Line' at cuana Arnold.
2.

If the tuo events listed are not designated as abnomal occurrences in this quarterly report, we suggest that they be ir.cluded as ever'ts dacted not to be AO's.

The NRC co=pleted the ctaluation and issued new technical specifications that li=it race of power cacalation and average fuci exposure. The licensee restarted the plant in accordance with the new technical specifications.-
Actions Tcken to Prevent Recurrence.
Licensoe - The licennee has implemented a more rigorous primary coolcat and of t-gas monitoring program to dercct any onset of fuel fo11ure.
Monitoring thasa indiencors and strict control of power cocalation raten should preclude gross failures.
N3C - The NRC is c1csely monitoring the licensee's program cnd evaluating the data.
The NRC's Office of Inspection and Caforcenant monitored the.
startup activitios and vili continue to moniter licenseo compliance uith the new technical specifications.
