SUPPLEMENTAL INFORMATION FOR LER NP-32-77.1 DATE OF EVENT: April 28, 1977 FACILITY: Davis-Besse Unit i ed IDENTIFICATION OF OCCURRENCE: Installing incere detectors in violation of Technical Specifications 3.9.2, 3.9.5 and 6.2.2e. Incore instrumentation was being installed without the direct supervision of a Senior Reactor Operator licensed person: there was no com=unication between Control Room and personnel at the refueling station; no audible source range indication in Containment and Control Room.

Conditions Prior to Occurrence: The plant was in Mode 6 with the Reactor Vessel i Head off.

Description of Occurrence: At 0830 on April 28, 1977, began installing incore I instrumentation. After the nineteenth incore detector was installed at 1035, it was recognized this action was in violation of Technical Specifications.

[ The work was i=cediately suspended and a meeting of the Station Review Board was called to review the event.

Designation of Apparent Cause of Occurrence: This event has been determined to have been caused by procedure inadequacy in IC 2001.04, "Incore Detector Handling" and Replacement".

Analysis of Occurrence: The effect on reactivity coefficient of inserting the incore detectors into the initial core is insignificant. It is therefore con-cluded that the event.did not result in a threat to the health and safety of the public or to station personnel.

Corrective Action: A modification was made to the procedure to meet the Technical Specifications and allow the work to continue.

Failure Data: No previous similar events have occurred.
