SUPPLEMENTAL INFORMATION FOR LER NP-33-77-15 1

DATE OF EVENT: July 6, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: The Containment Emergency Air Lock was not demon-strated to be operable by means of leak rate test within the required 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

Conditions Prior to Occurrence: The plant was in Mode 4 with Power (MWT) = 0 and Load (MWE) = 0.

Description of Occurrence: Opening of the Emergency Air Lock occurred on July 3, 1977, at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />. At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on July 6, 1977, the Containment Emergency Air Lock was declared inoperable due to failure to complete a leak rate test j

(ST 5061.05) within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> after the July 3 opening. One door, rather than the required two, was tested during this time period. The function of the leak rate test on the air lock doors is to ver' fy containment integrity.

DesignationofAppahentCauseofOccurrence: The cause of the occurrence can be attributed to personnel error.

The operators performing the Air Lock Leak Rate Test failed to follow the test procedure, vhich stated that both doors were to be tested.

The entire test was performed again, and the air lock declared operable at 0005 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> on July 7, 1977. The station then satisfied Surveillance Requirement 4.6.1.3(a) and was removed from the Action Statement of Technical Specification 3.6.1.3.

Analysis of Occurrence: Although the air lock was declared inope able for 65 minutes, containment integrity was maintained since the door whien had passed.

the Air Lock Leak Rate Test requirements remained closed until the air lock was again operable.

No danger to the health and safety of the public or station peri.onnel occurred, therefore, because no radioactivity was released, and containment integrity was l

preserved.

l Corrective Action: After ratesting, operability was returned to the air lock 65 minutes after it was declared inoperable. To prevent recurrence, a modification has been made'to further clarify the Air Lock Leak Rate Test Procedure. Addi-tionally, the operators performing ~the test will be re-instructed that both doors are to be tested.
