\O)s U . TOLEDO EDISCN COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-32-77-10 DATE OF EVENT: July 14, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Possible degrading of Component Cooling Water .

System.

3 Conditions Prior to Occurrence: The plant was in Mode 4, with Power (MWI) = 0 and Load (MWE) = 0.

Description of occurrence: On July 14, 1977, at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br />, during the removal of startup strainer on suction of Component Cooling Water (CCW) Pump 1-1, chunks of valve liners were found in the strainer. Since valves CC171, an 18 inch Fisher Butterfly and CC1467,an 18 inch Hammel-Da 1 Butterfly (Decay Heat Re-moval Cooler 1-1 Outlet Isolation Valves) were reported leaking past the seat the previous day, it was concluded that the larger sirat pieces of liners were from valves CC171 and CCl467. A small 3" liner which w.a also found in the

.( strainer was concluded as being from either CC1454 or CC1457, Spent Fuel Pool Heat Exchanger Temperature Control Valves.

This occurrence is being reported in compliance with Administrative Control Technical Specification 6.9.1.S(i) . The station was not in violation of any Technical Specification and did not enter into any action statements.

The strainer was reinstalled in CCW Pump 1-1, and the pump was returned to service to pick up any additional ubber liner which may be in the system. The valves CC171,'CC1467, CC1454 and CC1457 were scheduled to be removed from ser-vice to inspect liners and to make necessary repairs.

Designation of Aoparent Cause of occurrence: The exact cause of failure is unknown. Upon disassembly and inspection of valves CC1467 and CC171, the liner had no indication of being bonded to the body with an epoxy which was specified by the valve manufacturer. Improper alignment by construction personnel is a second suspected possible cause. --

1 Analysis of occurrence: There was no danger to the health and safety of the public or station-personnel as a result of this occurrence. The other compon-ent cooling water loop was operable during the cause of this occurrence. No aberrations have been encountered in that loop.

Valves CC1454 and CC1457 are presently being repaired by Maintenance personnel. These steps were taken to prevent recurrence, l
