FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Makeup Pumps 1-1 and 1-2 were inoperable.

Conditions Prior to Occurrence: The plant was in Mode 3, with Power (MWT) = 0 and Load (MWE) = 0. ,

Description of Occurrence: On August 29, 1977, at 1225 hours0.0142 days <br />0.34 hours <br />0.00203 weeks <br />4.661125e-4 months <br />, the Control Room operator observed that Makeup Pump 1-2 tripped. He then unsuccessfully attempted to start Makeup Pump 1-1. Beth Makeup Pumps were inoperable, in violation of

. Technical Specification 3.1.2.4.

Upon investigation, it was discovered that Makeup Pump 1-2 had tripped due to a blown fuse on Terminal 21R of Relay Cabinet 2825. This fuse supplies 120 V AC to the relay which provides the " Low Level" interlock to trip the pump on low level in the Makeup Tank. This relay,also provides the interlock for Makeup Pump 1-1 and thus it could not be started.

There were no redundant systems available and operable; therefore, E=ergency Procedure EP 1202.15, " Loss of Reactor Coolant Makeup", was initiated isolating Reactor Coolant Makeup, Letdown and Seal Injection. Control rods were inserted to 0% and Abnormal Procedure AB 1203.04, "Depressurization of RCS with only Safety Related Equipment", was initiated. .

Designation of Apparent Cause of Occurrence: The fuse supplying 120 V AC to the relay used to provide the interlock for both Makeup Pumps blew. The exact reason for the blown fuse is not known.

Analysis of Occurrence: There was no danger to the health and safety of the public or Station personnel. From 1225 hours0.0142 days <br />0.34 hours <br />0.00203 weeks <br />4.661125e-4 months <br /> to 1335 hours0.0155 days <br />0.371 hours <br />0.00221 weeks <br />5.079675e-4 months <br /> on August 29, 1977, Emergency Procedure EP 1202.15, " Loss of Reactor Coolant Makeup", and Abnormal Procedure AB 1203.04, "Depressurization of RCS with ouly Safety Related Equip-ment", were implemented and would have safely shut down the plant if a Makeup Pump could not have been restored.

Corrective Action: At 1335 hours0.0155 days <br />0.371 hours <br />0.00221 weeks <br />5.079675e-4 months <br /> on August 29, 1977, the fuse was replaced returning Makeup Purps 1-1 and 1-2 to service thus removing the, Station from, the Action State =ent of Technical Specification 3.1.2.4.

To prevent recurrence, Facility Change Request 77-288 was issued. This requests separate circuits for each Makeup Pump to prevent one fuse from rendering both i Makeup Pumps inoperable.

Failure Data: No previous similar events have occurred.
