d SUPPLC4 ENTAL INFORMATION FOR LER NP-33-77-3 DATE OF EVE'Tr: May 11, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Lost audible indication on source range detector.
Conditions Prior to Occurrence: The plant was in Mode 6 with the reactor vessel head off. No core alterations in progress.
Description of Occurrence: At 0624 hours0.00722 days <br />0.173 hours <br />0.00103 weeks <br />2.37432e-4 months <br /> on May 11, 1977, accidental removal of the source range detector's temporary audible indicator electrical cord from its socket; causing a momentary loss of power and audible indication in the Control Room, violating Davis-Besse TS 3.9.2.
Situation was rectified (within two minutes) by insertion of plug into socket.
Designation of Apparent Cause of Occurrence: A construction worker inadvertently
s pulled electrical cord from its power socket.

a Analysis of Occurrence:
Since no core alterations were being performed and a boron concentration of 2; 1800 ppm boron was maintained, the health and safety of the public or station persont.al was not threatened.
Correction Action: Insertion of plug into socket. This audible indicator was only temporary, is no longer in use, and will not be used in the future.
Failure Data: No previous similar events have occurred.
