DAVIS-BESSE UNIT ONE NUCLEAR PO*JER STATION SUPPLEMENTAL INFORMATION FOR LER NP-32-77-4 DATE OF EVENT: June 1, 1977 FACILITY: Davis-Besse Unit 1 CONDITIONS PRIOR TO OCCURRENCE: Plant in Mode 5.

Power:

(MWI) -0 (MWE) -0 Identification of Occurrence: Containment Radiation Indicator Monitor (RIM) 2004 in Safety Features Actuation System (SFAS) Channel 1 was tripped as per Technical Specification Action r19 of Table 3.3-3.

The bistable for containment radiscion in SFAS Channel I was inadvertently reset the following day.

Description of Occurrenes: On May 31,1977, at 1825 hours0.0211 days <br />0.507 hours <br />0.00302 weeks <br />6.944125e-4 months <br />, RIM 2004 in SFAS Channel 1 was turned off so construction could work on it (as per Technical Speci-

fication 3.2.2.1, Table 3.3-3 Action 9).

The violation occurred when on June 1, 1977, at 1237 hours0.0143 days <br />0.344 hours <br />0.00205 weeks <br />4.706785e-4 months <br />, a Control Room Operator reset the bistable for containment radiation in SEAS Channel 1.

On June 2, 1977 at 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br />, the condition was discovered.

Designaticn of Apparent Cause of Occurrence: The modul,e was incorrectly reset due to a personnel error.

Analysis of Occurrence:

Since three out of the four SFAS radiation monitors were still operable, the health and safety of the public or station personnel was not threatened.

Corrective Action: On June 2.1977, at 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br />, the condition was discovered, the Shift Foreman was notified and at 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br />, the bistable was again tripped and tagged to preclude recurrence.

Failure Data: No previous similar events have occurred.
