. Reportable Occurrence Violation of Technical Specifications - May 27, 1977 - 1700 hours0.0197 days
0.472 hours
0.00281 weeks
6.4685e-4 months
Dcvis-Besse No.1 Reactor Core - Mode 5 - Power (MWT) 0 - Load (WE) 0 Upon completion of ST 5031.02 in SFAS Channel 4, prior to replacing the cover on the Crouse-Hinds junction box, a loose output lead was discovered in the box. While tightening it, it momentarily touched the box. With the lead shorted to ground current rose rapidly simu-lating an increase in RCS pressure. HV-DH11 stroked closed at its setpoint of 433 psig. The Operator immediately tripped the decay 1

heat pump.

The insulation on the output lead had been stripped back further than necessary. Improper installation.

The short was only momentary, then the bistable was reset, HV-DH11 reopened, decay heat pump restarted, and 3000 gpm flow to the RCS re-establisted.

'd Region III, Bob Warnick, Duty Officer telephoned, May 28, at 1415 hours0.0164 days
0.393 hours
0.00234 weeks
5.384075e-4 months
.
