, The containment personnel and equipment hatches vere open for more than  !

t seven days during the ninth refueling outage. The hatches vete opened to support outage activities. The seven-day time limit for the personnel and equipment hatches was exceeded on October 13, 1989. Since the containment vall is considered to be a fire barrier, opening these hatches constitutes breaching a fire barrier.

Technical Specification 3.7.12 requires that, within one hour, either  ;

establish a continuous fire watch or verify the operability of fire detectors and establish an hourly fire watch patrol. Technical Specification action statement requirements vere met in that personnel vere present continuously when the hatches were open. Technical Specification l 3.7.12 also requires that breached fire barriers be restored to functional i status within seven days or a Special Report must be submitted Within the following 30 days. Due to the scope of the work in progress, it was not practical to close these hatches within seven days: therefore, this Special Report is being submitted.
