This refers to the meeting held on March 12, 1975, by Messrs. hnnieutt, Hayes, and LaDous of this office with you and others of your staff at your corporate offices relative to construction activities at the Md1=rA site authorised by Nuclear Regulatory Commission Construction Permits No. CPPR-81 and No. CPPR-82. h purpose of the meeting was to discuss the effectiveness of the Quality Assursace/ Quality Control Program for the Mid1 mad project, h meeting focused on the reinforcement steel spacing discrepancias identified in the remator building and your recent report to the NRC concerning the inadvertent omission of reinforcement steel in a concrete beam located in the anw414mry building. It is recogniwad that these matters were identified by you or your contractor; however, as discussed in the meeting, we are concerned with the weaknesses in the Midland Quality Assurance Program that permitted these problems to occur and to remain undetected by the first line of quality control. Also discussed during the meeting were the commitments made by both Consumers Power Company and the Nuclear Regulatory Commission in testimony before the Atomic Safety and Licensing Appeals Board, and the need for continued, effective management involvement to assure that those personnel involved in the construction, fabrication, and installation of the Mid1 mad Plant underscand the need for strict adherence to the imple-montation of your Qeelity Assorsnee Program.
With regard to isomes raised during this meeting, we understand that you intend tot (1) complete the safety evaluation and engineering review relative to reactor building reinforcement steel spacing dis-crepeneias identified by Consumers Power Company, (2) continue your review of the safety implications and reportability considerations concerning reinforcement steel omissions identified in the==414=7
