SUMMARY

OF MEETING WITH ARKANSAS POWER & LIGHT COMPANY AND BABCOCK A'ID WILCOX COMPANY (B&W) REGARDING SURVEILLANCE SPECI'IEN HOLDER TUBE FAILURES (SSHT)'AT ANO-1 On May 20, 1976, representatives of AP&L, B&W and the NRC staff met to discuss the reactor vessel surveillance specimen holder tube failures at AN001. A.ist of the meeting attendees is attached.

On March 19, 1976, the ANO-1 facility was shutdown to inspect the units SSHT's since failure of these tubes had occurred at several.E&W facilities (Three Mile Isisnd - 1, Rancho Seco and Oconee). The prelininary inspection, performed on March 25 and 26, revealed that at 'least two of the SSHT's were damaged to the extent that portions of the tube and specimen holddown mechanism had fallen to the bottom of the reactor vesselv. I e reactor was subsequently defueled; the internals were removed; and a comprehensive inspection, cleanup-and repair program was initiated.

N. Keyworth, B&W, opendd the meeting with an introduction and presentation of the SSHT failure chronology for ANO-1.

L. Bohn, B&W, presented a description of the SSHT damage, repair and recovery operations. Video tapes were shown of the March 25 and 26 inspection. A description was provided of subsequent inspection and cleanup of the reactor vessel and core internals. Video tapes also were shown of the vessel af ter the debris was removed.

The inspection indicated only small scratches and small dents in the inner vessel wall.

M. Pendernrass, AP&L, presented a description of the licensee's fuel inspection. Video tapes were shown of dekris removal fran the bottom grillage of the assenblies.

A question and answer period followed.

2-i The general conclusions of the meeting were:

1.

The staff accepted the licensee's inspection, cleanup and repair program.

2.

A report describing the licensen's inspection and cleanup program would be provided. The content of the repcrt would be discussed with the ORPM.

3.

The results of a weight comparison that, comparing the weight of recovered debris with the calculated weight of the as-built components, would be supplied in the report (#2).

The licensee concluded that the ANO-1 facility was safe to operate I

on the basis of their inspections, cleanup and repairs.
