Letter Phillips to Knuth October 31, 1973 STATION INVERTER MALFUNCTION Description of Incident On Friday, September 28, 1973, during the early A.M. hours an inverter malfunctioned due to a power wire burning open at the " crimp type" lug. The inverter switched to the alternate source with no interruption of output power.

Subsequent investigation on September 28, 1973, revealed that-numerous internal power lead wires were loosely crimped in this inverter as wall as the other five inverters which includes one spare. It was then evident that this was a warranty failure and the other inverters had potential possibilities of failure.

Safety Implications This incident occured during the pre-operational testing phase of the plant prior to fuel load. Consequently, neither the safety of the public nor tha'.: of plant personnel was compromised in any manner. As noted above, the inverter automatically switched to the alternate power source following the malfunction and there was no interruption in output s

power.

Corrective Action Taken A representative of the manufacturer (Solid State Controls, Inc.)

was sent to the field to investigate the problem and supervise repair work. All internal power lead " crimp type" lugs were examined and re-placement lugs were applied as required.
