Letter Phillips to Knuth s-llh IDENTIFICATION OF BREAKERS AS NON-0 Description of Incident During the latter stages of Hot Functional Testing, the pressurizer heater circuit breakers (Breaker Nos. B5144 and B6144) were replaced by non-Q breakers. These breakers are safety related in that they isolate the non-Q heaters from safeguards busses. Arkansas Nuclear One maintenance personnel were not aware that the items were "Q" in that identification as safety related devices was not apparent.
Safety Implications The replacement breakers, although not classified "Q" at the time of instal-lation, were manufactured to the same quality level as the original breaker and have since been redesignated as "Q".
Because of this, no threat to the safety of operation would have resulted from this specific misidentification l
incident.
V Corrective Action To prevent a recurrence, the plant procedures covering the issue of trouble tickets will be revised requiring the cognizant supervisor to indicate on the trouble ticket "Q" or "Non-Q" so that personnel will be aware of the equipment status and quality personnel can verify status. All personnel who have responsibility in this area have been made aware of this occurrence and of the proposed procedure change.
