DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-77-23 DATE OF EVENT: July 14, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Makeup Pump 1-1 removed from service to adjust suction valve.

Conditions Prior to Occurrence: The plant was in Mode 4, with Power (MWT) = 0,

, and Load (MWE) = 0.

Description of occurrence: At 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> on July 14, 1977, Makeup Pump 1-1 was removed from service to allow maintenance on the suction valve, MU 191. This valve is a three inch split stem Grinnell Diaphram Valve with a manual actuator.

Upon investigation, the Maintenance personnel found the stem of the valve was loose. They promptly tightened the stem and re-adjusted the travel stop lock nuts. At this time, the plant was in Mode 4, Hot Shutdown, which requires entry into an Action Statement of Technical Specification 3.1.2.4 if either pump was removed from service.

At 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br /> on July 14, 1977, Makeup Pump 1-1 was returned to service.

Designation of Apparene Cause of Occurrence: The cause of this occurrence is

apparently due to improper initial installation by construction personnel.

Improper installation during construction has been noted on other Grinnell '

Diaphram Valves. As a result of Toledo Edison Company's Quality Control surveil- l j'

lance and a training saminar by Grinnell which identified many problems, Toledo i Edison's Maintenance Department is currently performing a preventative mainten-ance inspection ~(MWO-77-1192) on all Grinnell Diaphram Valves to eliminate re-currence due to improper installation.

Analysis of Occurrence: Since one makeup pump was operable and was able to handle operational loads while the other was out of service, there was no danger to the health and safety of the public or station personnel.

l Corrective Action: The stem was tightened and the travel stop lock nuts adjusted. l The pump was returned to service within fif teen minutes. The maintenance in-spection is expected to prevent recurrence due to improper installation during l l

construction.

Failure Data: No previous similar events have occurred.
