Our letter of January 18, 1973, notified ycu of a situation where a significant nunber of piping spcols for ASME Section !!! piping syste=s for-the Davis-Sesse Nuclear Fever Station had been fabricated at the Kernersville Flant of ITT-G * --a

centaining tack velds perferred by velders not previcusly qua' * '* ad. This letter contains a su::ary of the activities centributing to this deficiency, the resolution of the deficiency, and corrective actiens taken to prevent recurrence.

ASME Secticn III piping for the Davis-Besse Nuclear Fover Station is being fabricated by ICT-Grinnell Corporatien at the ernersville Plant in North Ocrolina. 2echtel Corpora:icn, acting as agent fer Toledo Edicon, is perfer:ing reutine shcp inspection and surva*a-aa of activitiec at the Kernersville Flant involving this pipe fabrication.

During routine shop inspection and surveillance activities, it was found that sete tack welding during fabrication fit-up vas being per-ferred by fitters not qualified fer velding in accordance with Section IX cf the AS:2 Code. An in-depth audit felleving this discovery revealed that 113 cpeci pieces cf ASME Section III, Class 2,and 2h0 pieces of Class 3 piping vere involved. In addition, tvc spool pieces of ASME Section III, Class 1, piping vere tack velded by a velder qualified enly in the 1G pesition without positive verification that all tack velding was actually perferred in the 1G positica. Cf these spool pieces, 72 ASME Class 2 pieces and 97 Class 3 pieces had been shipped to the 'ob site.

Since all velds in AS::E Class 1 and Class 2 c.ir.e rec.uire radiography, these -ieces vere accepted on the basis of satisfactory radiographic examination of each veld involved.Section IX of the ASZE Code ter=its

' 'Mr. Frank Kruesi Page 2 February 12, 1973 The Authorized Code Inspector of the ASME has been avare of this situation and has signed the AS:2 partial data reports for these pieces involved, confir..ing that these pipe spcols meet the requirements of AS2 Section III.

Extensive precedures have been instituted by Grinnell at the Kernersville Plant to ensure that no velding is dene by unqualified personnel and that all precedures are strictly adhered to.

Zechtel Shop Inspectors are =ain-taining close inspection and surveillance on shop procedures and shop verk and Toledo Edison is conducting independent audits of the Kernersville Plant operations.

A report Co.1200 has been prepared by ITT-Grinnell concerning the subject situation amplifying the above infor:ation. Cne copy of this report is enclosed.
