REPORT NUMBER-83RI-0419-00003 i

In accordance with NUREG-0619 a detailed report must be submitted discussing the inspections performed on the Tecdwater Nossle Blend Radii and Bore Regnons (Inner Radius) and Nostle to Safe End Welds.

Specification SERI-M-489.1 (Inservice Inspection Ten Year Program) is in compliance with the requirements of Table 2 of NUREG-0619 which requires augmented volumetric examinations of these areas.

This report summarises the inspections performed since the previous report through RF03.

The volumetric examinations were performed by General Electric )

under contract to System Energy Resources Inc. (SERI) . In addition, surface examinations were performed on two nossle-to-safe-end welds j (N4C and N4D) by U.S. Testing Company under contract to SERI. The data reports for all examinations are available for review upon i request.  !

In an offort to provide the Nuclear Regulatory Commission with the j requested information, this report is being presented in the t following format. Each item listed in Paragraph 4.4.3.1(2) of r NUREG-0619 is addressed separately. {

i

a. Startup/ Shutdown cycles l
1) Total number of cycles at Grand Gulf 64, total includes f 26 scrams, 38 heatups and their respective cooldowns.  ;
2) Total number of cycles since the previous volumetric  !

examinations on the blend radiit 8, total includes 5  ;

scrams, 3 heatups and their respective cooldowns. l

3) Total number of cycles since the previous surface i f

examinations on the blend radiit 64, total includes 36

. scrams, 38 heatups and their respective cooldowns, f

4) Total number of cycles since the previous volumetric J examinations on the nossle-to-safe-end weldst

~

8, total includes 5 scrams, 3 heatups knd their respective  :

cooldowns. j

5) Total number of cycles since the previous surface [

examinations on the nossle-to-sofe-end weldst

- N4A and N4B; 8, total includes 5 scrams, 3 heatups l and their respective cooldowns.

- N4C,N4D,N4E and N4F; 61, total includes 26 scrams, 35 heatups and their respective cooldowns.

b. Summary of Methods Used and Results of Previous Inspections During RF03 volumetric examinations were performed on the l j nossle blend radii and bore regions (inner radius) and the i i nossle-to-safe-end welds. Surf ace examinations were performed  !

I on the outside surface on two nossle-to-safe-end welds (N4C ]

and N4D).  !

Refer to tables 1 and 2 (attached) for examination methods and  !

l dates examinations were performed for each nozzle. l t

Refer to previous report (SERI-0619-00001) for the methods i used and results of inspections performed prior to RF03. l

c. system changes and changes in operating Procedures l 1

The volumetric examinations performed during RF03 were the  !'

second examinations performed since the preservice L examinations. There have not been any system changes or  !

changes to operating procedures that would effect system i I

temperature or flow during this time period.

l

d. Inspection Resulta j RF03, remote volumetric examinations were performed on I

! Durinfxfeedwaternossleblendradiiandboreregicns(inner all a I radius) and all six feedwater nossle-to-safe end welds. i i

No indications were recorded for examinatione on the blend  !

radii and bore regions (inner radius) which required j evaluation in accordance with ASME Section XI. ,

I Won-relevant indications were recorded on the nossle-to-safe-end welds. These indications were evaluated and determined to l be caused by: I.D. surface geometry, geometry from thermal i sleeve positioning ridge and root geometry. ,

P Final analysis' of the ultrasonic examination data was  !

parformed and found to be acceptable in accordance with the  !

General Electric procedure for reviewing and analysing .

recordable indications (NDE-1) and SERI standard SERI-M489.2. l j During RF03 surface examinations were performed on the outside  ;

surface of two nozzle-to-safe-end welds (N4C and N4D) . The surface examinations resulted in no recordable indications.

e. Isakage Monitoring [

Grand Gulf has not installed any type of on-line leakage l monitoring system capable of detecting leakage through  ;

degraded seals on the feedwater sparger. j i

f. Information Regarding UT Crack-like Indications and any j subsequent PT Indications. ,

No UT crack-like indications were detected, thus no penetrant  !

testing was required.
