In accordance with NUREG-0619 a detailed report must be submitted discussing the inspections performed on the Tecdwater Nossle Blend Radii and Bore Regnons (Inner Radius) and Nostle to Safe End Welds.

Specification SERI-M-489.1 (Inservice Inspection Ten Year Program) is in compliance with the requirements of Table 2 of NUREG-0619 which requires augmented volumetric examinations of these areas.

This report summarises the inspections performed since the previous report through RF03.

The volumetric examinations were performed by General Electric

)

under contract to System Energy Resources Inc. (SERI). In addition, surface examinations were performed on two nossle-to-safe-end welds j

(N4C and N4D) by U.S. Testing Company under contract to SERI. The data reports for all examinations are available for review upon i

request.

In an offort to provide the Nuclear Regulatory Commission with the j

requested information, this report is being presented in the t

following format. Each item listed in Paragraph 4.4.3.1(2) of r

NUREG-0619 is addressed separately.

Summary of Methods Used and Results of Previous Inspections During RF03 volumetric examinations were performed on the l

i nossle-to-safe-end welds. Surf ace examinations were performed on the outside surface on two nossle-to-safe-end welds (N4C

1 The volumetric examinations performed during RF03 were the second examinations performed since the preservice L

examinations. There have not been any system changes or changes to operating procedures that would effect system i

temperature or flow during this time period.

Inspection Resulta j

I RF03, remote volumetric examinations were performed on Durinfxfeedwaternossleblendradiiandboreregicns(inner all a I

radius) and all six feedwater nossle-to-safe end welds.

i No indications were recorded for examinatione on the blend i

radii and bore regions (inner radius) which required j

evaluation in accordance with ASME Section XI.

I Won-relevant indications were recorded on the nossle-to-safe-end welds. These indications were evaluated and determined to l

be caused by:

surface geometry, geometry from thermal i

sleeve positioning ridge and root geometry.

P Final analysis' of the ultrasonic examination data was parformed and found to be acceptable in accordance with the General Electric procedure for reviewing and analysing recordable indications (NDE-1) and SERI standard SERI-M489.2.

During RF03 surface examinations were performed on the outside surface of two nozzle-to-safe-end welds (N4C and N4D). The surface examinations resulted in no recordable indications.

Grand Gulf has not installed any type of on-line leakage l

monitoring system capable of detecting leakage through degraded seals on the feedwater sparger.
