O APPENDIX A University of California at Los Angeles Los Angeles, California 90024 Docket Number 50-142 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on February 13-14, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of NRC license R-71 as indicated below:

a.

Technical Specification VIII.J. states in part "The facility shall be operated and maintained in accordance with appreved written procedures.

...The following types of written precedures shall be maintained:

.. 2.

Procedures which delineate the operator action required in the event of specific malfunctions and emergencies."

Technical Specification VIII.L.3.b, action to be taken in the event of an abnormal occurrence, states in part " Abnormal occurrences shall include...., the following:

...an uncontrolled or unanticipated reactivity change."

Technical Specification VIII.M.l. states in part "... reports shall be made to the NRC as follows:

(1) A report not later than the following working day (by telephone or telegraph...'

and a report within 10 days (in writing...) of:

Abnormal occurrences as defined in Section L."

Contrary to the above on January 25, 1980, during an uncontrolled 1

reactivity change no emergency procedure existed which delineated operator action on a dropped rod resulting in the failure tc report the abnormal occurrence and a return to full power operation before the cause of the occurrence had been verified.

This is an infraction.

b.

Technical Specification III.D.3. states "The neutron channels shall be calibrated against an independent measure of core power at intervals not exceeding 12 months."

Contrary to the above, the neutron channels were not calibrated between December 8, 1978, and January 9, 1980, a period in excess of 13 months.

This is an infraction.
