This is to request an extension of the date for completion of the activities related to screening of contractor employees detailed in your undated Immediate Action Letter received on May 5,1980 for the Davis-Besse Nuclear Power Station Unit No. 1.
As agreed between Mr. J. F. Donahue of your office and Mr. M. R. Polk of Toledo Edison during a telephone conversation on May 15, 1980, the revised personnel screening procedure will be implemented by May 30, 1980 and the QA
