MILLSTONE UNIT NO. 3 REPORTING OF DEFICIENCIES IN DESIGN AND CONSTRUCTION This letter provides written confirmation of a phone conversation held with your Mr. McGaughy on April 15, 1980. We have determined that the problem as discussed below is reportable under 10 CFR 50.55(e)(1)(iii) .

As reported to Mr. McGaughy on April 15, 1980, our engineer constructor for Millstone Unit No. 3, Stone and Webster Engineering Corporation, reported to Northeast Nuclear Energy Company that examination of the component cooling heat exchanger support steel weldements Indicated they were not in con formance with purchase speci fications. The component cooling heat exchanger support steel was fabricated by Tower I ron Works.

Visual receipt inspection noted unsatisfactory welds as per the fabrication s pe ci fi ca t ion. Tower I ron Works was informed and the material put on " hold."

This receipt inspection nonconformance date was May 29, 1979 The steel supports for the three heat exchangers were installed under risk release No. 104 in August 1979 30/7 s

in nonconformance and disposition report No. 0169 The unacceptable welds found in the supports of the reactor plant component cooling heat exchangers Indicate possible loss of the function of the heat exchangers should the welds fall . The consequence of the loss of the heat i exchangers would be an inabili ty to maintain the plant in cold shutdown.

For these reasons, i t is being reported as a significant deficiency.

Disposition details indicate directions for rework. All rework will be done on site or new 'tems fabricated off site in accordance with the original s peci fi ca tions. Final disposition will have repaired or replaced the supports. Final disposi tion date is not known at this time.
