On at least three occasions in the past year, fractures have occurred at the weld between the dome and the box of source he-ds used in Kay-Ray Gauges, Model s 7050, 70508, 7051, 70518, 7060, 70608, 7061 and 70618.

These weld fractures caused the dome to separate from the box exposing the 500 millicurie cesium-137 sealed source.

In one case, the source fell loose and was completely exposed.

To correct the potential hazard, Kay-Ray has developed a support bracket which they will supply free of charge to affected licensees which is 3

intended to prevent dome separation if a fracture occurs. On June 30, j

1978, Kay-Ray wrote to affected customers noti fying them of the avail-aoility of the support bracket and asking them to return a card confirming j

possession of the source heads (Enclosure A). A number of Kay-Ray custo-mers have not returned the card.

Ninety-two customers are located in Agreement States and subject to Agree-ment State jurisdiction (Enclosure B).

Because of the incomplete response to Kay-Ray's June 30, 1978 letter and tne potential radiation hazard which may result if the units are not modi-fied, tRC is contacting all affected customers within NRC's jurisdiction through IE Bulletin No. 78-13, dated October 27, 1978 (Enclosure C).

The Eulletia contains a form to be returned to NRC and a card to be mailed to Kay-Ray i f the source is still in possession of the licensee.

We ask that each of the Agreement States havino affected Kay-Ray customers within their jurisdiction to do at least the f:llowing: Write to each customer in your State notifying them of the potential hazard; ask each custcmer to take the actions recuested in the IE Bulletin and obtain confirmation that the actions have been taken.
