Unit Identification and Description of Occurrence:

Relay 16AK-27 coil failed causing RWCU isolation valves FCV-69-2 and FCV-69-12 to close.

FCV-69-1 failed to close.

Conditions ~ Prior to Occurrence:

Unit 1 @ 98%

Unit 2 0 77%

Unit 3 @ 99%

Action specified in the Technical Specification Survedlance Rettuirements met due to inoperable equipment.

Describe.

NA due to FCV-69-2 remained closed.

Aoparent Cause of Occurrence: The cause of the relay coil failure was normal W ing and deterioration w1Mit expected life.

FCV-69-1 failed to close due to a loose wire, internal to the MOV, which also had a sajority of strands broketi due to mechanical wear. The wire was the common wire between the close limit switch and the close torque. switch, furnished by limitorque.

Analysis of Occurrence:

There was no damage to plant equipment.

There was no activity release, no personnel exposure or injury and no danger to the health or safety of the public.

Corrective Action

The relay coil was replaced and the wire relugged and terminated. The procedure for limit switch adjustment has.been revised to include verification of termination integrity.
