In Ine second paragraph under 2, " Quality Assurance Program," change the commit-ment initiation from "after the effective date of the QA Program" to "after January 1, 1979."

Under 6. " Quality Assurance Records," provide commitments that:
a) The records will be maintained in accordance with a written procedure.
b) The records will be identified and retrievable.
c) The Radiation Safety Officer will maintain a list of the records and their storage location.
