SOUTH TEXAS PROJECT ELECTRIC GENERATING STATION RESPONSE TO NRC INSPECTION FINDING DOCKET NOS. 50-498/80-01 AND 50-499/80-01
Dear Mr. Seidle:
The following is our response to the item of noncompliance identified in IE Inspection Report Nos. S0-498/80-01 and 50-499/80-01, dated March 2S, 1980 (received by Houston Lighting 5 Power Company on March 28, 1980).
Failure to Follow Test Procedure Corrective Action The bulged section of the liner shall be repaired in two basic steps:
Step 1 - The bulged section of the liner shall be removed.
Step 2 - The section of liner shall be replaced, and subsequently grout shall be installed behind it.
Recurrence Control The liner bulge was created by additional pressure which resulted becsuse the control skid (with the monitoring pres-sure gauge) was located approximately twenty feet above the injection point on the liner.
This created head pressure in addition to the gauge pressure.
Therefore, the recurrence control is to:

Locate the control skid within ten feet of elevation of the injection point (s) on the liner.

Maintain continuous observation by QC of the gauge pressure on the control skid and at the injection point (s) during both water and grout injection.
3.
Use a gauge and a shutoff valve at the point of injection.
4.
Maintain direct communication between the pump and the control skid.
Date Action to be Achieved 4
The date action is to be completed is October 3, 1980.
Liner removal is presently scheduled for the week of April 21, 1980.
