Dear Mr. O'Reilly:

'.On April 22, 1980, M. D. Quinton, South Carolina Electric and Gas Company's (SCE&G) ruclear Engineering, notified NRC-0IE Region II, Resident Inspector Jack Skolds, of a substantial safety hazard under the provisions of 10CFR21 regarding the failure of an adaptor in the air start modification package for the Ey))~ The diesel generators are manufactured by Colt Industries - Fairbanks Morse Engine Division of Beloit, Wisconsin. This is the final report on the subject reportable deficiency.

On April 11, 1980, hile ._ " .111ng the air start modification supplied to us by Colt Industries an the 'B' EDG, a Colt-Pielstick Type PC-2 Diesel Generator, a male to female adaptor, part number P12610490, group 12, assembly 138, that goes into the lubricating oil system was broken off. ,

'. Investigation by Nuclear Operations Technical Support Personnel revealed that the cause of the failure was due to insufficient wall thickness be*~een the male and female parts of the adaptor to support the load exerted on it during tightening. The load exerted on the fitting was not excessive during tightening as evidenced by the fact that the adaptor failed before the copper ga.=ket the adaptor seated on v; c compressed. Further investigation showed that less than 1/32" wall thickness existed between the male and female parts of the adaptor.

This condition is attributed to excessive drilling of the adaptor while preparing the f emale part for threading. An examination of the same adaptor supplied for the 'A' EDG revealed the same condition as that found with the adaptor for the 'B' EDG.

The safety i= plication of this deficiency, had it gone undetected, could have been to cause the failure of both EDG due to loss of lubricating oil. The loss of both EDG, concurrent with loss of offsite power, could preclude nur ability to achieve a safe shutdown condition.

Corrective action taken has been by SCE6G Nuclear Operations Technical Suppo'rt Group to engineer and fabricate a new adaptor for each EDG.

It is recommended that purchasers and/or licensees that have implemented or are going to implement similar modifications to a Colt Diesel Generator visually inspect fittings fabricated by Colt for conditions similar to those found in the male to female adaptors supplied to the V. C. Su=mer Nucleat Station, Unit 1.
