Gentlemen:

In response to the subject report, the following is provided.

NOTICE OF VIOLATION:

Based on the results of an NRC inspection conducted during the period of January 22 through February 21, 1980, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your license (DPR-51), as indicated below:

10 CFR 20.203 (c) "High Radiation Areas" states, in part, "Each entrance or access point to a high radiation area shall be:... Equipped with a control device which shall energize a conspicuous visible or audible alarm signal in such a manner that the individual entering the high radiation area and the licensee or a supervisor of the activity are made aware of the entry: or... Maintained locked except during periods when access to the area is required, with positive control over each individual entry."

10 CFR 20.202 (b) (3) defines high radiation area as "any area, accessible to personnel, in which there exists radiation originating in whole or in part within licensed material at such levels that a major portion of the body could receive in any one hour dose in excess of 100 millirem."

Mr. G. L. Madsen April 3, 1980 1-040-02 Contrary to the above, on February 6,1980, door 194 was found to be unlocked and open with no positive access control.

Door 194 provides access to the Unit 1 North Piping Penetration Room, which was a posted high radiation area with accessible whole-body radiation in excess of 700 millirems per hour.

This is an infraction (313/80-03-01).

RESPONSE

TheWaste Control Operator has the responsibility to make a check of all locked doors each shift. All WC0's have been reinstructed in this responsibility.

Additional corrective steps which will be taken to achieve full compliance are:

1) Adjust the alarms on all the doors so that it is not possible to clear the " door open" alarm in the control room unless the doors are fully shut.

2)

Install a sign on all doors to high radiation areas instructing persons entering to notify the control room upon entering nd exiting the room.

3) Administratively require the Shift Supervisor to have the WC0 check the doors shut if a door alarms and item 2 is not compiled with.

These controls will be in place and full compliance achieve by 5/27/80.
