Failure occurred during SI with unit at 53% power.

Teflon sheet failed providiiig' no protection to rupture dise under vacuum condition.

Cause of the teflon sheet failure is unknown, but is believed to be due to normal a

fatigue.

TheTe was no sigifificAnt resulting chain of events and no danger to health or safety of the public.

Corrective Action

Rupture dise was replaced. Approval has been received to change to an improved type of rupture disc. The discs will-be replaced when system conditions allow.
